Making an appointment

Please have your referral and x-ray or scan reports sent through to our email reception@footandankleqld.com or fax 07 3831 0049.

If you do not have access to fax or email please contact your referring doctor to send this information on your behalf.

Dr Lutz will review and triage your referral according to the urgency of your condition. Our office will then contact you with an appointment date. If you do not hear from us one week after sending your referral, please phone our rooms.

Once an appointment has been booked with you, you will receive an email confirming all the details and referrals for further investigations if they are required. Please make sure to bring the results of these investigations to your appointment.

• Your doctor referral
• X-rays and/or scans on film only – Dr Lutz is not able to view CD’s
• Current Medicare card
• Health Fund details
• Workcover claim details (if applicable)
• DVA Card details (if applicable)
